Providence Health MCG Data Breach

MCG Health LLC is a vendor of Providence Health & Services regarding electronic patient files. On March 25, 2022, MCG learned that it experienced a data breach.

Providence Health & Services patients may have had their confidential and protected health information accessed and stolen.  Information subject to the breach includes: full names, Social Security numbers, medical codes, postal and email addresses, telephone numbers, dates of birth and genders.

Notice was mailed to some individuals affected by the data breach on June 27, 2022.  If you received notice, or otherwise believe you were part of the data breach, you may be entitled to compensation.
